Affordable Care Act (ACA) Form 1095-C Requests
Changes to the Law for Furnishing Forms
Employees and former employees may request copies of Affordable Care Act (ACA) Form 1095-C tax forms by submitting the request by email to benefits@wcps.k12.md.us or mail to 10435 Downsville Pike Hagerstown, MD 21740.
The law changed and now allows employer flexibility for furnishing these forms. Forms will not be printed and mailed to home addresses like they were in the past.
Requests will be processed in accordance with federal requirements, and documents will be provided within the applicable time frame. Please allow a reasonable processing period after submitting your request. Forms will be provided within 30 days after the request date.
Electronic Forms through Employee Self-Service (ESS)
Employees may elect to receive their 1095-C form electronically through ESS:
- Go to the ESS Site.
- Log in using your username and password.
- Click “Personal Information” on the left side of the screen.
- Click “Tax Delivery Options” in the upper right corner of the screen.
- Select “Self Service Only” under 1095 Delivery Method.
Log out of ESS.
Forms will be available starting March 2, 2026.
Use of the Form
This form is intended for employees or former employees to verify they had health insurance coverage for the 2025 year. This form is not needed to file your taxes.
